“The future of the QALY: replacement or revision”
Chris Sampson is a Senior Principal Economist at the Office of Health Economics, where he is the research theme lead for Value, Affordability, and Decision Making. Chris is an expert in economic evaluation, with a particular interest in mental health and digital therapeutics. His research spans a broad range of topics in health policy and economics. Chris is a member of the EuroQol Group and leads on the development of several 'bolt-on' items for the EQ-5D. He is currently the Lead Convener for IHEA’s Mental Health Economics SIG and is also the founder of The Academic Health Economists’ Blog.
The quality-adjusted life year (QALY) metric, pivotal in health care decision-making across numerous countries, faces increasing scrutiny due to methodological criticisms and anti-rationing lobbying efforts. This Healthy Policy Seminar will first consider various proposed alternatives to the QALY, such as Health Years in Total (HYT) and the Equal Value of Life Years Gained (evLYG), assessing the suitability of these metrics as replacements for the QALY. Then, the session will address an important flaw in current approaches to the estimation of QALYs—specifically, the challenges in valuing 'dead' states—and argue for a fundamental revision to the interpretation of QALYs. The discussion will underscore the irreplaceability of QALYs, while advocating for continuous methodological revisions to their estimation, refinement of their interpretation, and clarity on their role in decision-making.
